Your First Visit
Your first visit experience will depend on whether you’re coming to us for general dentistry or orthodontic treatment. We ask that you fill out our new patient forms beforehand and bring them in with you. That way, you won’t have to worry about spending time filling them out in our waiting room. If you’re coming in for a checkup and cleaning, you’ll meet with Dr. Aquino, but if you’re visiting us for an orthodontic consultation, your appointment will be with Dr. Lai. If you have any specific questions about your upcoming consultation or appointment, reach out to us.
New Patient Forms
Before your first appointment, we ask that you download and fill out our new patient forms. This makes the initial process faster, and it keeps you from having to shuffle through insurance documents in our waiting room. If you have any difficulty accessing the paperwork or you need help filling it out, feel free to call our staff.
Dental Insurance Welcome
At White Oak Dental & Orthodontics, we’re proud to accept all PPO dental insurance plans and file claims on our Woodbridge patient’s behalf to maximize their benefits. We’re also an in-network provider with most major plans in the area. We currently do not accept Medicaid or NJ Family Care.

In-Office Membership Plan
For our patients who don’t have dental insurance or who would rather utilize a simpler, easy-to-navigate plan than dental insurance, we also offer an in-house membership plan. After paying a low annual or bi-annual rate, you can have your basic preventive needs covered, including exams, X-rays, and cleanings. This plan also gives you discounts on other services you receive, even cosmetic procedures, which are not typically covered by insurance.
